Report into 2003/2004 New South Wales (Australia) hospital safety released

New South Wales – 452 serious incidents that occurred during 2003 and 2004 have been brought to light, by a report released today about the safety of patients in the hospital system in New South Wales, Australia. Many of the incidents highlighted by the report resulted in injury, or even death.

The report detailed 13 patients who had operations that they were not meant to have or who had the wrong part of the body operated upon. It also noted that another 22 were seriously injured in falls in wards and bathrooms.

Other statistics included four suicides in hospital, with another 128 occurring in community care. Another nine people retained surgical instruments or materials inside them after the operation. The rest of the serious incidents related to labour and delivery problems (26) and clinical management problems (157).

Pakistan court sentences one man to death penalty, and life imprisonment to five others for Mashal Khan lynching incident

On Wednesday, an anti-terrorism court in the Pakistani city of Haripur sentenced one man to death, and five others to life imprisonment, for killing 23-year-old Mashal Khan in a mob violence incident last year. 57 out of 61 verdicts were announced, which took place behind closed doors for security reasons, and about 250 police officials and commandos were deployed.

Imran Sultan Mohammad, who had confessed about shooting Mashal Khan, was sentenced to death by judge Fazal-e-Subhan. Khan was a journalism student in Mardan’s Abdul Wali Khan University, who was accused of blasphemy, an offense punishable by death in Pakistan, and was killed in mob violence on April 13. Later in June, police investigation revealed it was a false accusation as they found no evidence of blasphemy. Khan was stripped, beaten by dozens of people in the boys hostel, thrown from the second floor and shot dead — which was videographed and posted on social media.

Defense lawyer Saad Abbasi said 25 accused were sentenced to three-year prison, and 26 accused were released free from charges. Mashal Khan’s brother Aimal Khan said, “Our demand was that all of the suspects should have been convicted […] We ask the police to arrest the remaining suspects who are still at large and to bring them to trial.” Police had arrested one suspect recently, who is yet to face the trial.

Last year after the death of Mashal Khan, speaking to BBC Urdu, his father Iqbal Khan said, “This issue is not just about my son. The issue is the mob came to the university, and challenged the government. So the government should question themselves, or do justice.” He had also said, “In this country, freedom of speech, those who speak the truth, is restricted. They cut people’s tongue. According to police findings, the false accusation of blasphemy was started by a student group after Mashal Khan had criticised the university for hiking the fees, asserting corruption in the university. Some students had complained about Khan’s alleged secular views to the university authorities.

Reportedly around a hundred relatives of accused waited outside the premises, for the verdict. “A day will come that the judge will answer the God. The verdict he has announced is unjust”, said a guardian whose son was sentenced to three years’ jail.

Pakistani Prime Minister Nawaz Sharif condemned the lynching last year and said, “Let it be known to the perpetrators of this act that the state shall not tolerate citizens taking the law in their own hands. No father should have to send his child off to be educated, with the fear of having him return in a coffin.”

Utah bill requiring divine design education withdrawn by Senator Chris Buttars

Utah Senator Chris Buttars has decided not to introduce a bill requiring the teaching of “divine design” in Utah schools, at least for this year. Buttars withdrew after State Board of Education director Patti Harrington assured him that Utah public school curriculum does not teach that man descended from apes.

Buttars had hoped his divine design proposal would escape the controversy of creationism or intelligent design. “The only people who will be upset about this are atheists,” he stated on June 6 when he announced his intention to run the bill.

Utah is home to the Church of Jesus Christ of the Latter Day Saints also known as the Mormon Church. On the surface it would seem Utah would be a likely scene of intense religious pressure in public schools over the teaching of evolution. But unlike states with a strong Christian conservative presence, Utah’s LDS leaders have avoided some of the more contentious separation state and church battles.

On the topic of divine design, official LDS church spokesmen have been largely silent at least in public. But with more than 90% of the legislature made up of members of the LDS faith, few in Utah would deny the influence of the church on public policy.

Groups on both sides of the issue are gearing up for what many consider an inevitable fight. The ACLU of Utah has posted a paper on divine design on its web site (http://www.acluutah.org). The Eagle Forum which wields significant policital power in Utah has expressed its support for Buttars proposed legislation.

Congressman Cunningham admits taking bribes

U.S. Representative Randy “Duke” Cunningham (RCA) pled guilty today to conspiring to take bribes in exchange for using his influence as a member of the House Appropriations Committee to help a defense contractor get business. In total he pled guilty to one count of income tax evasion and four counts of conspiracy, namely mail fraud, wire fraud, bribery of public official and accepting bribes. U.S. District judge Larry A. Burns scheduled Cunnigham to be sentenced on February 27. He is facing up to 10 years in prison and nearly $500,000 in fines, as well as forfeiture of unspecified amounts of cash and property.

In the court hearing, Cunningham admitted to accepting “bribes in exchange for performance of official duties” between “the year 2000 and June of 2005”, taking “both cash payments and payments in kind” and following up by “trying to influence the Defense Department”.

The federal investigation against Cunningham was triggered by his sale of his California residence to defense contractor Mitchell Wade in late 2003. However, Wade never moved in and sold the house at a $700,000 loss three quarters of a year later. At the same time Wade’s company MZM won tens of millions of dollars in defense contracts. Subsequent investigations discovered more questionable business transactions, including interactions with the defense contractor ADCS. In his plea agreement he testified that, among other charges, he “demanded, sought and received at least $2.4 million in illicit payments and benefits from his co-conspirators in various forms, including cash, checks, meals, travel, lodging, furnishings, antiques, rugs, yacht club fees, boat repairs and improvements, moving expenses, cars and boats.”

Cunningham announced his resignation after the hearing. In a written statement released by his law firm O’Melveny & Myers LLP he declared “The truth is — I broke the law, concealed my conduct, and disgraced my high office. I know that I will forfeit my freedom, my reputation, my worldly possessions, and most importantly, the trust of my friends and family.”

The Next Consortium of ITRI launches ‘RoboScooter’

At the Pavilion of Taiwan Automotive Research Consortium during the AutoTronics Taipei 2008, in Taipei Time, the Next Consortium of Industrial Technology Research Institute (ITRI) unveiled a brand-new environmental-concept vehicle named “RoboScooter”. It was jointly designed by the Creativity Lab of ITRI, SYM Motors, and the Media Lab of Massachusetts Institute of Technology. This announcement was approved by industrial, governmental, and academical executives, but not just in Taiwan.

The ability for R&D to manufacture the electronic vehicles and bicycles was concerning to several automobile-related industries. There was concern not only from several trade shows, but also international competitions related to the design after winning several prizes back in Taiwan to establish the industrial position and brand image of “Made in Taiwan”. Since several industries like automobile are transformed to high-value industries, “creativity” progressively became a key element before unveiling any new products.

As of this sector, Wen-jean Hsueh (ITRI Creativity Lab General Director) hoped companies could do proper transformations on its R&D nature from “Made in Taiwan” to “Created in Taiwan” for best practice. This not only echoed “OBM” theory discussed at 2007 Taiwan Brands’ Trend Forum but also blended with a key element of production – “creativity”.

On the other side of SYM Motors, to solve the transportation problem for TWTC Nangang, SYM Motors Taiwan imported manufacturing technology from Kinglong Bus Co., Ltd. to manufacture several 4th-cycle low floor buses and sell them to Chung-hsing Bus Co., Ltd. and Kwang-huang Bus Co., Ltd. In this launch, the SYM Motors Taiwan hope this vehicle is energy-efficiency, easily-parked, and has creative functions which can improve the transportation habits of individuals.
